2016-2017 Fall Sports Program "Shout outs"

Attached is the order form for "Shout Outs" for the 2016-17 Fall Sports Program. These programs are sold during the Varsity football games and display all Fall team sports photos as well as the band. They also include a color photo of all Seniors participating in a Fall sport, cheerleading, or band.

Anyone interested should follow the directions on the attachment. Completed forms are to be returned to Glenn Evans at the Mayfield Athletic Boosters. The deadline for all submissions is 8/12/16.
