June 2nd, 2015 - Fall Band Item Order Sheet

Please see the Fall Band Items order sheet for all remaining purchases for this coming fall. In order to have everything ready by the start of Band Camp, we process our orders over the early summer. On the attached document, please make sure you order/purchase (or already have) everything for your respective instrument (see first page "Things You Need to Have." All completed orders (with payment - payable to Mayfield Band Boosters) must be returned to the High School in an envelope with your name on it. You can mail the envelope or drop it off in the office. Either drop it off or mail it, attention to Mr. Oshaben.
